Seat Belt Save
A contractor driller received minor injuries when the truck he was driving experienced engine failure while driving up a 40% grade road.  The truck rolled backward, over a berm and barrel-rolled an estimated 10 times.  The driller was wearing his seat belt.

Golden Star Arm Amputation Accident
EX10 cylinder was taken out and secured on a wooden block on top of the stick for the welder to take off the bushings.  The first bushing was taken off and just as the second was being washed, the cylinder came off the wooden block, dropping on the right arm of the welder.

Fatal Crane Accident
During the construction activity of lifting and moving the steelstructure (trestle) that supports the conveyor at the Alegria 8/9 stockpile to a new location, the crane lost stability, rolled over onto it sides coming to a stop against a section of a conveyor gallery that was being pre assembled adjacent to the crane location.

Lost Time Injury Announcement
Three workers entered the partially choked discharge chute of a scalping screen and positioned a ladder via an access point between the conveyor and the bottom of the chute.  Whilst the cleaning activity was in progress, starting from the top, the build-up became dislodged and trapped 2 of them.

A Dash 3 continuous miner (CM) was being moved out of a heading when it slewed unexpectedly and trapped a mine worker, who was acting as a cable hand, between the rib and the head of the CM, causing life-threatening injuries.

Fatal Crush Injury on Exploration Site
A water truck operator sustained fatal injuries when he was crushed between the rear of his water truck and a pastoral property gate.  The operator had opened the gate, driven through it and had just closed it when the truck rolled back trapping him.  Preliminary investigation, testing and mechanical inspection of the truck has shown that the brakes were ineffective.
